Information you choose to share

Wellform LLC collects the information you submit through the workshop-interest and class-feedback forms. This may include your name, email, topic preferences, attendance preferences, and class feedback. Responses help Anne improve her classes and plan future workshops and resources. Forms are processed through Netlify Forms. Please avoid including sensitive health information. Feedback is reviewed and used publicly only with your explicit testimonial permission.

Your choices

Your email is not published. Providing an email or selecting a topic does not subscribe you: workshop invitations are sent only if you select the separate invitation checkbox. Testimonial permission is also optional and separate. You may opt out of invitations by replying to an invitation email. You may request deletion of your information or removal of a published testimonial at any time.
